Terms and Conditions of Use

PLEASE READ THESE TERMS AND CONDITIONS CAREFULLY BEFORE USING MYCHART. This website (Site) and related services are provided by ProHealth Care (PHC) and Health Care Providers sharing the PHC electronic medical record to its patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of this Site will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit this Site.

Should you chose to sign up for MyChart, or participate in virtual visit, your use of such web pages and services provided in such online forums may be subject to additional terms. In the event of a conflict between such additional terms and these Terms and Conditions of Use, the additional terms will control.

Changes to these Terms and Conditions of Use. We may revise and update these Terms and Conditions of Use from time to time in our sole discretion. All changes are effective immediately when we post them. Your continued use of the Site following the posting of revised Terms and Conditions of Use means that you accept and agree to the changes.

The information available through MyChart may be protected health information (PHI) as defined under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and its implementing regulations, and may be protected by HIPAA and other state and federal laws governing the privacy, security and confidentiality of medical records, PHI and other personal information. PHC will maintain the privacy of your confidential medical information and will not disclose medical information unless you authorize the disclosure of your information or the disclosure of the information is permitted by law. Please review our Notice of Privacy Practices for a complete description of how we gather, use and protect your confidential medical information.

You are solely responsible for any sharing (e.g., via email, text message, or other communication methods) of your MyChart account and the information available through MyChart, intentionally or unintentionally, to other people or other entities, including health care providers and outside vendors of mobile applications or other products and services.

Relationship of MyChart to Your Medical Record Information

MyChart is a communication tool offered to you only as a convenience. You may receive messages, test results, and other communication through MyChart. MyChart provides online access to portions of the patient electronic medical record stored in our Epic electronic health record; the entire patient medical record is not available through MyChart. Access to the complete patient medical record may be requested by contacting our Health Information Management Department.

Electronic Messaging

MyChart should never be used for urgent health care matters. The typical turnaround time for response to electronic messages is one to two business days. If you believe you are experiencing an urgent or life threatening health condition, please call your physician’s office, go to the emergency room of a local hospital or dial 911.

If your provider is out of the office or unavailable, messages sent via MyChart may be viewed by other health professionals who work with your physician; therefore, use of My Chart may not be appropriate for sensitive information you wish to discuss directly with your provider.

Health Care Providers may send you messages via MyChart. It is your responsibility to monitor these messages and respond in an appropriate manner.

You agree that we may use all telephone numbers you provide to us to communicate with you by telephone (including cell phone and text messages) regarding your account, appointment reminders, bill payment reminders, debt collection efforts and other information and marketing messages. Further, by clicking “ACCEPT”, you confirm that the telephone number provided is your telephone number, and you authorize us to contact you with marketing and other information by contacting you at the telephone number(s) you have provided.

Secure Communications

All communications between you and Health Care Providers using MyChart occur over a secure connection. You are required to provide a valid e‐mail address, and it is your responsibility to keep this address updated if you change your email account. You will receive internet e‐mail messages notifying you that new information is available in your MyChart account. Although these email messages will not contain any confidential medical information, the notice that new information is available by accessing MyChart may be information that you would not want others to know. This could include your family members, employers or anyone else who can access your e‐mail account. Please take this into account when providing an e‐mail address or using MyChart. All of the confidential medical information available to you in MyChart is protected and securely maintained by Health Care Providers.

Access Code

Before you are able to access your records in MyChart, the system requires confirmation of your identity. One identifier that is verified is your Social Security number. Once verification is completed, your Social Security number will not be used again by MyChart.

When you initially enroll in MyChart, you will be provided with an activation code. The activation code will give you the opportunity to create a personal login name (called MyChart ID) and a password. Once this activation code has been used, it will no longer be valid. It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information, read your messages, and send new messages as if that person were you. It is your responsibility to prevent disclosure of your login name and password, and to change your password if you feel that its security has been compromised. You may change your password at any time. We shall not be liable or responsible for any damages that result from your failure to keep your password secure.


Parents or legal guardians may create and have full access to a MyChart account for their minor child up to age 11 and partial access for minors age 12 to 17. Teenage patients age 14 to 17, with consent from their parents or legal guardians, may create and access their own MyChart account. Parents or legal guardians, with the consent of their teenage child age 14 to 17, may create and access their teenage child’s MyChart account. Adult patients (age 18 and over) may open a MyChart account and allow another adult access to that account by filling out an Adult Proxy form. Permanent legal guardians of adult patients may create and access an adult patient’s MyChart account.

There are certain types of health care information that the parent or legal guardian of a minor patient age 12 or older may not view without the consent of the minor patient. Because of these requirements, MyChart does not contain all of the patient’s health care information.

If your legal relationship with the patient changes, you agree to immediately inform ProHealth Care by calling 262-696-5843, or sending written notice to ProHealth Care, Health Information Management Department, N17 W24100 Riverwood Drive, Suite 200, Waukesha, WI 53188, and you agree to immediately stop accessing the patient’s MyChart account.


At this time, the following MyChart web portal features are available:

  • Ability to review, request, schedule or cancel your appointments.
  • Ability to view selected portions of your medical record such as allergies, medications, histories, problem list and recent visit information.
  • Ability to check in for appointments prior to your arrival.
  • Ability to make payments online.
  • Ability to schedule and participate in a virtual (video) visit with a Health Care Provider.
  • Ability to receive selected results.
  • Ability to communicate with your health care provider via secure messaging regarding health concerns.
  • Ability to request prescription refills.
  • Ability to link to other sites that will provide you with health information.

Termination of Access

You may cancel your enrollment in MyChart at any time by completing a MyChart Inactivation form. ProHealth Care reserves the right to deny, restrict or revoke access to MyChart at any time if your use of MyChart violates these Terms and Conditions. You will be notified if we discontinue your MyChart access.

Accuracy and Integrity of Information

Although Health Care Providers attempt to ensure the integrity of the Site, it is possible that the website could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy is identified, you should inform your health care provider so that it can be corrected. Information contained on the Site may be changed or updated without notice.


MyChart may not always be available to you due to system maintenance, backups or failures. Access is provided on an “as is available” basis, and you are not guaranteed access at any particular time.

Accessing the Site and Account Security

We reserve the right to withdrawal or amend this Site, and any service or material we provide on the Site, in our sole discretion without notice. We will not be liable if for any reason all or any part of the Site is unavailable at any time or for any period. From time to time, we may restrict access to some parts of the Site, or the entire Site, to users, including registered users.

Both federal and Wisconsin laws govern the confidentiality of patient health care records. We are dedicated to keeping your medical information confidential. Our workforce is subject to strict standards for maintaining the confidentiality of your medical information. The Site places considerable emphasis on the security of medical information. MyChart has been set up to be as secure as possible from unauthorized access. By allowing you access to your information, you must share in this responsibility. Health Care Providers and the website cannot be held responsible for:

  • Unauthorized disclosures resulting from a user not logging out of an active session.
  • Unauthorized disclosure resulting from a lost or stolen user ID and password.
  • Unauthorized disclosure resulting from information printed by the user from MyChart.
  • Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security.
  • Similar events outside the substantial control of Health Care Providers or the Site.

Links to Other Sites

Health Care Providers may create links on the Site to other web sites that we think may be of interest to you or provide further information. We make no representation whatsoever about any website that you may access through this Site. When you access a non‐Health Care Providers site, it is independent from Health Care Providers and Health Care Providers have no control over the content of that website. Health Care Providers do not endorse or accept any responsibility for the content or use of any linked site. If you decide to access any third party website linked to this Site, you do entirely at your own risk.

You will be notified when you are leaving the Site. We feel it is important to make this distinction, as we have not reviewed the privacy policies of any websites linked to or from the Site, and you should exercise care when visiting any other website. We would like to hear from you if you find any websites linked to or from the site objectionable.

Your Right to Object

If you believe Health Care Provider(s) is/are using your personal identity information in a way that is not consistent with the Terms and Conditions, you should contact us at once.
